Resident Asessment and Preliminary Service Plan

A health assessment is a required document for all residents wishing to move into or who already reside in an Adult Family Home or private home in Washington State. The assessment must be done and contain accurate information about the resident’s current needs and preferences. It is used as a tool to get to know the resident and their care-planning needs. The provider must be knowledgeable about the resident’s current needs and preferences before an AFH provider can admit a resident to their home. 

AFH providers will use the preliminary service information along with the assessment to create a Negotiated Care plan within 30 days of admission. The AFH provider must ensure the assessment contains the minimum information required as listed in WAC 388-76-10335. The preliminary service plan includes the resident’s specific problems, needs, goals, & preferences. It includes how safety & health will be promoted by meeting care needs & handling refusal of care.

The assessment must be updated at least every 12 months, when there is a significant change in the resident’s physical or mental condition, when the negotiated care plan no longer reflects the resident’s status, needs, or preferences, and at the resident or resident’s request.   

I, Candace McGill, RN BSN ND, am a qualified assessor in Washington state as defined in WAC 388-76-10150. I am a bachelor-prepared RN from Washington State University.  I have over a decade of experience in the home healthcare field, working with patients of all ages.  My focus areas of nursing have been palliative care, Home Health, ICU, Emergency Dept, Caregiver Education, and I am a DSHS-approved Nurse Delegator.  I am the owner of Pacific Frontier Consulting Services. We offer Long-Term Care consulting for Adult Family Homes and Assisted Living Facilities, caregiver and staff training, family caregiver training, resident assessments, and nurse delegation for both private pay and DSHS clients. 

The information contained in this assessment was obtained by an in-person interview with the client.  It may also contain information gathered from medical records, pharmacy records, family & caregiver report, and any other valuable resource of information.

 If you believe any information to be incorrect, missing, or invalid, you must notify Pacific Frontier Consulting within 1 week of the assessment date.  Any changes to the assessment after that may require an additional fee.
