Reiki Class Registration

Check if you have taken a Reiki Training before. If you have, please indicate what level of Reiki you have studied, with whom, and what year it was.

Please indicate if you have had an organ transplant, have a pacemaker/defibrillator or other electronic implanted device, or if your doctor has advised against increasing your immune system function, or circulation.

Do you require any special accommodations (i.e. pregnant, use a wheelchair, unable to lay on stomach or back, sensitivity to light, music, smoke, etc.)? If so, please explain:

Private classes are scheduled between you and Sarah during the week (generally a Tuesday, Wednesday, or Friday from 10:00-3:00, or 1:00-6:00). Note the independent learner option does NOT include additional mentoring sessions between classes, whereas the supported learner option does.

Each option requires a $100 deposit to be submitted before your space is held. You may pay the remaining balance at part one or in three increments (one at each part).

After filling out the registration form, you will also be invited to our online booking portal. Once you have an account, this will allow you to store a card on file (required) that can be charged for the deposit, remaining balance, or increments of the class.

Please note, that your spot is not offially held until a deposit is received and this registation form is submitted.

I agree to have a credit card on file. This is required for all clients, even if you plan to pay by cash or check at the time of class. After registering, you'll be invited to create an account & add card on file, or Sarah will call you directly to do so.
