Clinic Policies:
1. Each patient requires an up to date, detailed health history including home address, phone number, DOB. As well as all relevant health history in order to maintain health record & provide safe treatments. Health records are maintained electronically.
2. Assessments and interviews are conducted each visit to ensure safe and effective treatments are being provided as Health Care Professionals. This is included in your appointment time.
3. Forms of payment: Debit, Cash, E-transfer (sent by end of visit) and direct billing to eligible insurance plans. Only the patients Primary plan is billed at the office. Patients are responsible for billing additional benefit plans themselves.
4. If you are ill, you MUST cancel. Please give 24 hours notice to avoid late cancellation fees. Patients who disregard this policy & attend while ill will be discharged from care (or while there is a contagious illness in the household including but not limited to: Covid, Flu, Cold, GI bugs, Strep, HFM, Pink Eye, etc). Please report all illnesses before the 24 hour window to avoid paying for your missed appointment.
